since you seem to like hml  i will present you the easy way ( i frame)
Create a simple html page ala (your code).:

this is the code i put into source code:-

  <option value="saab">Discord</option>
  <option value="fiat">Forum</option>

<input type="text" name="mail"><br>
<input type="text" name="comment" size="50"><br><br>
<input type="submit" value="Submit Ticket">
<input type="reset" value="Reset">

Save this one (on root or somewhere as eg. main.html ???)

Now go create a page in e107 (use wysiwig < source code)

PASTE : (where you have to point to your location of file)

 [html]<p><iframe width="600" height="450" style="border: 0;" frameborder="0" src="http://domain/mail.html"> </iframe></p>[/html]

just that > save 

well, do not just copy that little piece, but adjust it..by copy and test it will likely throw some error because it is a fake address..
Look at the outcome and you know what you get...(or need to change > in html file)
